Selection of osteotomy is decided by careful preoperative assessment of the patient and deformity and the amount of correction needed to have a balanced spine. MC: "Osteotomy" literally means "to cut bone." A "column" is roughly 1/3 of the width of the spine. Thus, the goal of three-column osteotomies is to break the spine in half without damaging the spinal cord. First, the back portion of the spine covering the spinal cord, known as the lamina, is removed. A spine with too much kyphosis (forward curvature, or “hunchback”) or too little lordosis (inward curvature, or “swayback”) can be corrected with a PSO. During a pedicle subtraction osteotomy, the shaded areas of bone are removed. The spine is then realigned and stabilized in its new alignment.
